Nametag Request

Please complete each field in the form below. Submissions with blank fields will not be processed.

All Clarke University nametags follow the format below:

Clarke University
NAME (including degrees and certifications)
TITLE (official title as stated in your contract or employment letter)

Nametags are available in two sizes:

Standard Nametag: 3.25 x 1.75″
$11.50 each
Required for all faculty and staff

Small Nametag: 1 x 3″
$11.00 each
For approved staff and students

Note: Nametags are ordered once per week. Submitted nametag requests must be received by noon on Thursdays. Requests submitted after this time will be included in the order for the following week. Please allow up to 1 week for your nametag(s) to arrive from processing date.
